Actual Pilot Interview Experience for Republic Airways

Flew to IND on USair pass sent by Republic, e-ticket, self check-in, and was give a seat at the gate. Everything and everybody was very friendly and helpful in respect to the travel.
There are at least three Holiday Inn Selects in Indy, you want the one near the "Pyramid" north of town.  Republic has made transportation to the Hotel via Layfette Limo that picks up outside the transportation center outside of baggage claim. It was at least one hour waiting and the ride is at least 25mins.

The 25 question ATP type Test and Wonderlic Test are first. Everything was straight forward. The following questions I remembered:
Is MDA agl or msl? MSL
What is the FAF on the precision approach? GSIA
Wake Turbulance : which configuration increases wake turbulance? slow clean heavy
What speed flown to planned alternate? normal cruise
What are the lowest ILS mins with cl&TDZL? 200 DH & 1800rvr
When will you see straight in mins for the LDA? 30degrees or less from centerline
